SUBSTANTIVE REQUIREMENTS -7- May 21, 1992 <br /> FOR DDRW, SHARPE <br /> any immediate damage to the environment, any corrective/cleanup actions taken and/or <br /> additional monitoring proposed. <br /> 10. The Discharger shall submit to the Board, the Department and the US EPA monthly <br /> operation reports by the 15th day of the following month. These operational reports shall <br /> contain a summary of the operating parameters, O&M activities, and a summary of any <br /> shutdown or spill events that occured during the month. <br /> 11. Prior to beginning full-scale operation, the Discharger shall submit to the Board, the <br /> Department,and the US EPA the results from a ground water contaminant transport <br /> model. The model results must be for 3 snapshots in time during the first year after <br /> beginning of FSO. The model results, from each snapshot in time, must have maps or <br /> plans that display the following: <br /> (a) Anticipated draw down or mounding at each of the monitoring wells used for <br /> monitoring SHARPE's extraction and injection activities. <br /> (b) Anticipated plume concentrations at each of the plume monitoring wells presented in <br /> a concentration contour map. <br /> 12. Nine months after beginning full-scale operation of SHARPE's GWTS, the <br /> Discharger shall submit to the Board, the Department and the US EPA, a system <br /> evaluation report. The system evaluation report shall determine if the performance <br /> monitoring well network provides sufficient hydrologic and water quality data to <br /> accomplish the following objectives: <br /> (a) Monitor water levels and water quality within the permeable zone screened by the <br /> extraction wells and within overlying and underlying zones to show that the <br /> extraction system maintains hydraulic control of the VOC plume that is greater than <br /> 5 parts per billion. <br /> (b) Verify that the capture zones of individual extraction wells overlap or that the <br /> extraction system maintains hydraulic control of the entire plume,to the maximum <br /> extent feasible. <br /> (c) Determine hydrogeologic and water quality parameters of the ground water regime. <br /> (d) Monitor water levels and water quality around the injection wells to determine if the <br /> discharge of treated ground water significantly affects ground water contamination <br /> migration patterns. <br /> (e) Operate the ground water extraction and injection system in the most effective <br /> manner. <br /> 14. The Discharger shall comply with the "Standard Provisions and Reporting <br /> Requirements for Waste Discharge Requirements",dated 1 March 1991, which are <br />
