5. All records pertaining to each employee, including training records <br /> are kept in the Civilian Personnel Office. <br /> a. A separate file is kept for each employee containing the following: <br /> (1) Employee name <br /> (2) Job title <br /> (3) Written job description <br /> (4) Job qualification requirements <br /> (5) Training <br /> (6) Work history, including leave, awards, etc. <br /> b. The initial 24-hour, eight-hour refresher and hazard communication <br /> training is documented or a Department of the Army (DA) Form 1556-1 , signed <br /> by the instructor, and placed in the employees' file located in the Civilian <br /> Personnel Office. <br /> (1) Supervisors of employees involved with hazardous waste <br /> operations are responsible for maintaining training cards (file) and insure <br /> all employees receive training as required. <br /> (2) Emergency response supervisors shall certify that each covered <br /> employee has attended and successfully completed the required training or <br /> shall certify the employee' s competency at least annually. The method used <br /> to demonstrate competency for certification shall be recorded and maintained <br /> by the supervisor. <br /> 6. The Federal Government maintains all employee personnel records <br /> indefinitely. <br /> a. Records on current personnel are kept, in the Civilian Personnel <br /> Office. <br /> b. Records on former personnel are maintained in place for three (3) <br /> years. The records are then forwarded to the National Personnel Center, St. <br /> Louis, Missouri , where they are kept indefinitely. <br /> B. Number and Deployment of Employees <br /> The responsibility for safe operations and maintenance of the hazardous <br /> waste storage Building 530 and 605 lie with the wastewater treatment <br /> operators and the Depot' s Environmental Program Manager. The foreman of the <br /> Wastewater Treatment Section will be the main person responsible while four <br /> additional operators will be involved in the operations of the hazardous <br /> waste storage areas. <br /> The operators receive on-site and on-the-job training and attend a <br /> hazardous handling course, taught on-site by a Certified Training Instructor <br /> for Hazardous Material Handling and Transport. <br /> The remainder of the employees receiving training under this plan are <br /> distributed throughout the facility.
