• The OHSC, in coordination with the subcontractor health and safety representative, <br /> will provide, to the PM, an Accident/Incident Report (Attachment D to this HSP) that <br /> includes the following <br /> ' • A description of the emergency (including date, time and duration), <br /> • Date, time and names of all persons/agencies notified and their response, <br /> ' • Date, time and names of all persons/agencies notified and their response, <br /> • A description of corrective actions implemented or other resolution of the <br /> incident, <br /> ' • All workers on site are responsible for conducting themselves in a mature, <br /> calm manner in the event of an accident/unusual event All personnel must <br /> conduct themselves in a manner to avoid spreading the danger to themselves <br /> ' and to surrounding workers <br /> ' 3.0 RESPONSIBILITIES <br /> The OHSC will have responsibility for directing response activities in the event of an <br /> ' emergency He or she will <br /> • Assess the situation <br /> • Determine required response measures <br /> ' • Notify appropriate response teams, <br /> • Determine and direct on-site personnel during the emergency <br /> 1 <br /> The OHSC will coordinate the response activities of on-site personnel with those of public <br /> ' agencies <br /> ' 4.0 PUBLIC RESPONSE AGENCIES <br /> ' A list of public response agencies to be contacted and who may, depending on the nature of <br /> the situation, assume authority for emergency response is included in the site-specific HSP The HSP <br /> ' presents local emergency numbers, including local hospitals (which include poison control centers), <br /> the local health department, ambulance service, fire and police departments and others Contacts at <br /> ' the U S Environmental Protection Agency USEPA and the Poison Control Center also are listed In <br /> M - 2 <br />
