Contaminated groundwater pumped out of the excavation shall be containerized <br /> for proper disposal. <br /> After the EHD has approved the well destruction scope of work proposal and the public <br /> participation comments have been properly addressed, you may submit an EHD Well and Boring <br /> Permit Application form along with a Master File Record form, encroachment permit issued by the <br /> City of Stockton, access agreement from the adjacent property owner where an off-site well is <br /> located, and appropriate fees for each parcel. The permit fee for the destruction of each well is <br /> $125, along with a $375 Inspection Activity fee per parcel. A review of the site reports indicate <br /> that there are twenty-seven monitoring wells and six vapor extraction wells; for permitting <br /> purposes, nested wells are considered as one well (seven nested monitoring wells and three <br /> nested vapor extraction wells), yielding a total of 23 well destructions to be permitted. If this is a <br /> correct well count, a fee of $3,250.00 (23 wells x $125 + $375 x 1 parcel) will be required for the <br /> well destruction permits.
