SUMMARY OF VIOLATIONS CLASS I,CLASS II,or MINOR-Notice to Comply) Routine hazardous waste inspection done today. 101. This facility does not have a valid EPA ID number to manage hazardous waste. A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately obtain an EPA ID number to manage hazardous waste and submit evidence to the EHD by July 18, 2013. A form has been provided. 108. By the fence along the East border of the property, there are three unlabeled five gallon buckets contain unknown contents. At least one of these five gallon buckets is full of oil. Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately make a hazardous waste determination for these buckets of unknown contents, and manage it according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed by July 18, 2013. 122 & 125. Copies of hazardous waste disposal records for 2010, 2011, 2012, and 2013 were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD by July 18, 2013. 128. A maintenance record by HMC dated November 6, 2012 indicate that fuel filters from the dispensers were changed out but do not indicate where they were disposed. Bills of lading for the recycling of used ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
