A form is provided that can be <br /> used for this purpose. Submit proof of correction to the EHD by July 18, 2013. <br /> 703. At the time of inspection, it could not be demonstrated (not necessarily documented) that <br /> employees who handle hazardous waste were properly trained. When Charanjeet Kaur was asked <br /> about fuel spill clean up procedures, she stated that she didn't know. The generator must ensure that all <br /> employees who handle hazardous waste are thoroughly familiar with proper waste handling and <br /> emergency procedures. Immediately provide training to all employees who handle hazardous waste and <br /> submit a copy of the training records to the EHD by July 18, 2013. <br /> 812. On February 17, 2011, a routine hazardous waste inspection was conducted. This facility was cited <br /> for having a bundle of universal waste lamps left outside leaning against the wall near the dumpster and <br /> that on the ground outside next to the dumpster lay the broken remains of the a universal waste lamp. <br /> When Charanjeet Kaur was asked about the disposal records for the universal waste lamps, she stated <br /> that she did not know. A UW handler shall keep a record of each shipment of UW sent from the UW <br /> handler's facility to another facility. The record may take the form of a log, invoice, manifest, bill of lading <br /> or other shipping document. The record for each shipment of UW sent shall include the following <br /> information: <br /> (1) The name and address of the UW handler or destination facility to which the UW was sent; <br /> (2) The quantity [count or weight, consistent with, for example, section 66273.32, subsection (d)] of each <br /> type of UW sent (e.g., batteries, thermostats, lamps, electronic devices, CRTs, CRT glass); <br /> (3) The date of departure of the shipment of UW. <br /> A UW handler shall retain each record for at least three years. Submit a statement and supporting <br /> documentation explaining how this waste was managed by July 18, 2013. <br /> 9999. EHD has not received a response to the inspection report issued on February 17, 2011. CA HSC <br /> section 25185 (c)(3) The operator of the site or facility which receives an inspection report pursuant to <br /> paragraph (2) shall submit a written response to the department or the local officer or agency authorized <br /> to enforce this chapter pursuant to subdivision (a) of Section 25180 within 60 days of receipt of the <br /> inspection report, or within a shorter time as the department or the local officer or agency may <br /> reasonably require, which shall include a statement documenting corrective actions taken by the <br /> operator or proposing corrective actions which will be taken by the operator, for purposes of compliance <br /> with this chapter. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by July 18, 2013. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT O REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> EHD Inspector: A r4, Received By: Title: <br /> Aris Cacapit (209) 468-3453 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
