CAMBRIA <br /> Work Tasks <br /> Permit. An appropriate permit for well destruction will be obtained from SJCEHD. <br /> Site Safety Plan: Cambria will prepare a Site Safety Pian for field work. <br /> Utility Clearance: Cambria will mark proposed drilling Iocations and the locations will be <br /> cleared through Underground Service Alert prior to drilling. <br /> Well Destructions: The groundwater monitoring wells (MW-1 through MW4) and vapor <br /> extraction wells (VEW-1 and VEW-2) will be destroyed by pressure grouting. This method <br /> involves filling the well casing with cement grout from the bottom up. Once the casing is filled <br /> with grout, a pressure seal is placed on top of the well casing and additional cement grout is <br /> pumped under pressure into the well casing. This forces the grout through the well screens, out <br /> into the filter pack material. Then the wellhead vault boxes will be removed and the remaining <br /> hole .will be backfilled with concrete to grade. A Cambria geologist will supervise well <br /> destruction activities. <br /> Report Preparation: Following completion of well destruction activities, Cambria will prepare a <br /> written report documenting these activities. <br /> The scope of work described in this work plan will be performed under the supervision of a <br /> registered geologist or engineer. <br /> Schedule <br /> Cambria is prepared to begin work upon written approval of this work plan by SJCEHD and <br /> receipt of an appropriate drilling permit.
