If you have filed an extension past 8/15/92, we will need a copy of the extension application and your 1988 tax returns. 2. Please indicate who can sign for both the corporation and the partnership. 4. Please be aware that the State requires that you put at least 10% of the total cost into the project. I understand you have incurred and paid some expenses that the State may accept as your portion of the 10%. Please send an explanation/description of these expenses, the amount, and the date they were incurred/paid. If the State does not accept all of these expenses or they are not sufficient, please include were the additional funds are to come from to complete your portion. 5. Please send the Permit to Construct, when you have received it. 6. You included a 7/14/92 and 8/23/91 manifests from hazardous waste removal for oil. Are you only removing these annually? The State (Department of Commerce) is now requiring the "three most current manifests". Please send the three most recent. In addition, you did not include any manifests for used antifreeze disposal. Please send the three most recent. 7. The State is now requiring a copy of the complete application to the State Water Resources Board. Please submit this with the above information. I have enclosed a checklist for your convenience. The highlighted items are what I still need to process your request.
