Metal grindings wer observed in an unlabeled plastic container under the brake grinding <br />machine, and uncontained urrounding the machine. All hazardous waste containers shall be marked <br />with the following informati n: <br />1. the words "Hazardo s Waste" 4. physical state (liquid, solid) <br />2. name and address o generator 5. composition (contents) <br />3. hazardous propertie (flammable, corrosive, reactive, toxic) 6. accumulation start date <br />Immediately label these wa tes, or place them in a properly labeled container, and ensure that all <br />hazardous waste container are marked with all the required information. <br />REPEAT VIOLATION <br />610. A 55 gallon black steel drum of used oil filters was observed without an accumulation start date. <br />According to the disposal records on site, filters were last hauled away on June 12, 2012. Used oil filters <br />must be stored in a closed, rainproof container, labeled as "drained used oil filters," and marked with an <br />accumulation start date. I mediately mark the date on the drum and ensure that all used oil filters are <br />managed in this way. Sinc it cannot be determined how long this waste has been on site, immediately <br />recycle these filters under a bill of lading and submit a copy of the bill of lading to the EHD by September <br />27, 2013. <br />REPEAT VIOLATION <br />701. A 25 gallon gray ste I drum of metal fines was observed in the shop with an accumulation start <br />date of July 18, 2011. Facil ties who generate less than 1000 kg of hazardous waste per month and do <br />not exceed 6000 kg of wast stored on site at any time may store waste on site up to 180 days. <br />Immediately contact a licen ed hazardous waste hauler to dispose of this waste under manifest and <br />submit a copy of the manife t to the EHD by September 27, 2013. <br />REPEAT VIOLATION <br />703. An emergency coordinator and modified contingency plan information is lacking. There must be <br />at least one emergency coo dinator on site or on call to coordinate emergency response measures, and <br />the following information mu t be posted by a phone: the name and phone number of the emergency <br />coordinator; location of fire xtinguishers, spill control equipment, and if present, fire alarm; and the <br />phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br />emergency coordinator and �ost the required information by a phone. A form is provided that can be <br />used for this purpose.
