CONTINUATION FORM <br />OFFICIAL INSPECTION REPORT <br />Facility Address: 1645 Cherokee, Stockton (IRC) Page 4 of 4 <br />#80/82/83 Observe an unlabeled waste circuit board in a metal trash can by <br />the outdoor workshop. Universal Waste -Electronic Device shall be stored <br />in a manner that prevents release to the environment, labeled to identify <br />the type of Universal Waste (i.e. Universal Waste -Electronic Devices), and <br />stored on site no more than 1 year from the date it became a waste. <br />Immediately dispose of the above circuit board in accordance to Title 22 <br />Universal Waste Rule. Continue to labeling all universal waste in the <br />future to identify the type of Universal Waste with the date when it first <br />became a waste. <br />#89 5 metal 55 -gallon empty drums were not marked with the date they were <br />emptied. Empty HW or Hazardous Material containers larger than 5 <br />gallons must be marked with the day it was emptied and managed within <br />one year. Immediately manage the above empty containers by reusing, <br />recycling, or disposing them, and continue to mark all empty containers as <br />above in the future. Submit evidence of correction to this office by <br />January 8, 2010. <br />Notes: <br />The following were corrected on site: <br />■ Labeled Hazardous Property on the HW label of the used oil container. <br />■ Disposed the 5 -gallon bucket of used oil under the loader to the used <br />oil container. <br />■ Posed Emergency Coordinator Information <br />■ Disposed absorbent pad (unknown if it is HW) in a trash bin and <br />managed as HW. <br />Submit by January 8, 2010 a copy of the following: <br />1. A completed RETURN TO COMPLIANCE CERTIFICATION (see <br />back of the Inspection Report) <br />2. A written statement of how each of the above violations was <br />corrected. <br />3. All documents, photos, and receipts verifying the corrections. <br />Please be aware that all EHD staff time associated with failing to comply by <br />the above noted dates will be billed at the current hourly rate $115. <br />This facility is subject to re -inspection at any time at EHD's current hourly <br />rate. <br />EHD Inspector: Jeffrey Wong REHS <br />Received by: By Mail <br />
