The contingency plan must include: 1. Description of actions facility personnel will take In response to fires, explosions, or any Sudden or f7 non -sudden release of hazardous waste to air, soil or surface water at the facility; !/ 2. Description of arrangements made with local police departments, firs departments, hospitals, contractors and State and local emergency response teams to coordinate emergency services; + 3, list of names, addresses, and phone numbers (ofFce/homelcell) of all persons qualified to act as emergency coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and all others in order in which they will assume responsibilities; 4. List all emergency equipment, which Includes location, physical description, and brief outline of Ib Capabilities; 5. Evacuation routes for facility personnel where there Is a possibility of evacuation; 8.
