Update contingency plan and submit to this office by June 10, 2016. <br />Violation 110, CCR 66265.16(a -c). This is a repeat violation. Failed to complete <br />training on hazardous waste management and emergency response procedures. <br />Employees at this facility were unable to properly handle hazardous wastes as witnessed during <br />the inspection and they stated they have not been trained on hazardous waste management. <br />(a)(1) Facility personnel shall successfully complete a program of classroom instruction or on- <br />the-job training that teaches them to perform their duties in a way that ensures the facility's <br />compliance with the requirements of this chapter. The owner or operator shall ensure that this <br />program includes all the elements described in the document required under subsection (d)(3) <br />of this section. <br />(2) This program shall be directed by a person trained in hazardous waste management <br />procedures, and shall include instruction which teaches facility personnel hazardous waste <br />management procedures (including contingency plan implementation) relevant to the positions <br />in which they are employed. <br />(3) At a minimum, the training program shall be designed to ensure that facility personnel are <br />able to respond effectively to emergencies by familiarizing them with emergency procedures, <br />emergency equipment, and emergency systems, including where applicable: <br />(A) procedures for using, inspecting, repairing, and replacing facility emergency and monitoring <br />equipment; <br />(B) key parameters for automatic waste feed cut-off systems; <br />(C) communications or alarm systems; <br />(D) response to fires or explosions; <br />(E) response to ground -water contamination incidents; and <br />(F) shutdown of operations. <br />(b) Facility personnel shall successfully complete the program required in subsection (a) of this <br />section within six months after the date of their employment or assignment to a facility, or to a <br />new position at a facility. Employees hired after the effective date of these regulations shall not <br />work in unsupervised positions until they have completed the training requirements of <br />subsection (a) of this section. <br />(c) Facility personnel shall take part in an annual review of the initial training required in <br />subsection (a) of this section. <br />A checklist outlining the requirements for Personnel Training Requirements for Hazardous <br />Waste Generators was provided to the owner by email on October 30, 2015. <br />Train all employees who handle hazardous waste in hazardous waste management <br />and provide updated employee training to this office by lune 10, 2016. <br />Violation 111, CCR 66265.16(d). Failed to maintain complete personnel training records <br />on hazardous waste management and emergency response procedures. <br />Complete training records were not available for all hazardous waste employees. <br />(d) The owner or operator shall maintain the following documents and records at the facility: <br />(1) the job title for each position at the facility related to hazardous waste management, and <br />the name of the employee filling each job; <br />Page 11 of 39 <br />
