A written training program and training records were not available on site. Facility personnel shall complete a training program in accordance with Title 22 regulations. This program shall be directed by a person trained in hazardous waste management procedures and shall include the following: -- hazardous waste management procedures, including contingency plan implementation -- emergency procedures, equipment, and systems -- key parameters for automatic waste feed cut-off systems -- communications or alarm systems -- response to fire, explosion, or groundwater contamination incident -- shut down of operations The training program shall be completed by each employee within six months of hire, prior to unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until closure of the facility and shall include: -- name & job title of each employee -- written job description for each position (job title) listed, including requisite skill, education, qualifications, and duties -- description of training to be given to each position -- records that training was completed for each employee Immediately prepare and implement a complete hazardous waste management training program for this facility in accordance with Title 22 regulations and ensure that all employees complete this training at least annually. Submit a copy of the training program and records to the EHD by February 20, 2011. 60, 61, & 62. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
