(continued) <br />If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date <br />that the waste was accepted by the initial transporter, the generator shall submit an Exception Report <br />that includes a legible copy of the missing manifest, and a cover letter signed by the generator <br />explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports <br />shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. <br />By February 20, 2011, submit to the EHD a copy of the signed off copy of the manifests or a copy of the <br />exception report submitted to DTSC. <br />60 & 63. Copies of hazardous waste disposal records for used oil between June 18, 2010 and <br />December 7, 2010 were not found on site. Prior to July 2010 a record of used oil disposal is available <br />for each month. No disposal records were available for used antifreeze. Hazardous waste generators <br />shall retain copies of all manifests signed off by the disposal facility and all receipts used in a <br />consolidated manifesting procedure on site for three years and have them readily available for review. <br />Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, <br />and submit copies to the EHD by February 20, 2011. <br />83. Used fluorescent bulbs were observed unlabeled in the upstairs storage area. A 55 gallon drum of <br />aerosol cans was observed labeled as hazardous waste without an accumulation start date. A <br />generator shall label or mark universal waste with the words 'Universal Waste' and the type of waste <br />(alkaline batteries, mercury containing equipment, fluorescent bulbs, electronic devices, CRTs, aerosol <br />cans). Immediately label these wastes and ensure that all universal wastes are labeled with the required <br />information. <br />84. Facility was unable to demonstrate the amount of time used fluorescent bulbs and aerosol cans were <br />stored on site. A universal waste generator shall accumulate universal waste for no longer than one <br />year from the date the waste was generated.
