Ms. Stacy Rivera <br />San Joaquin County EHD <br />immediately, there is a possibility that the reserve diesel UST system may be red <br />tagged to prevent fuel inputs. REPEAT VIOLATION <br />Ryder Fuel Services contracted with Balch Petroleum on January 19, 2011 to <br />replace the sensor. Balch obtained the necessary permit and will be on-site to <br />replace the sensor on March 2, 2011. Copies of the work order summary will be <br />forwarded to the EHD upon completion. <br />#16 Violation Text: Liquid was observed in the main diesel (12,000 gallon) fill <br />sump, reserve diesel (6,000 gallon) fill sump, main diesel STP sump, and used <br />oil fill sump. If water could enter into the secondary containment by <br />precipitation or infiltration, it must be removed and disposed of properly. This <br />liquid was removed and managed by the technicians on site at the time of <br />inspection. Ensure that all sumps are maintained free of liquid. REPEAT <br />VIOLATION <br />The liquid was removed by the technician on the day of the inspection. Site <br />personnel have been instructed to maintain the sumps for all storage tanks. <br />#17 Violation Text: The designated operator failed to document all the alarms from <br />the attached alarm history on the March 2010 and November 2010 designated <br />operator monthly inspection reports and failed to check that they were <br />responded to appropriately. The missing alarms include: February 23,2010 (L7 <br />- new oil annular, L9 - new oil STP sump, L 15 - oil transition sump) and <br />November 7,2010 (L 18 - used oil fill sump). During the monthly inspection, the <br />designated operator shall review the alarm history for the previous month, <br />check that each alarm was documented and responded to appropriately, and <br />attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information <br />on the reports. REPEAT VIOLATION <br />Ryder Fuel Services has instructed the designated operator to include copies of <br />the alarm history print-outs with the monthly checklists. Upon receipt of the <br />checklists from the designated operator, Ryder Fuel Services personnel will <br />review the alarm history report and ensure all alarms are noted with the proper <br />response. <br />#18 Violation Text. Annual monitoring system certification, leak detector testing, <br />and spill container testing was performed on January 25,2010 and the test <br />report was submitted to the EHD on March 15, 2010. A copy of the test report <br />must by submitted within 30 days of the test. Take all necessary precautions to <br />ensure test reports are submitted in a timely manner. <br />
