The training records on site did not include a written job description for each position listed. Facility <br />personnel shall complete a hazardous waste training program within six months of hire, prior to <br />unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br />closure of the facility and shall include: <br />- name & job title of each employee <br />- written job description for each job title listed, including requisite skill, education, qualifications, duties <br />- description of training to be given to each position <br />- records that training was completed for each employee <br />Immediately updated the training records and maintain an updated copy of these records on site at all <br />times. Submit a copy of the updated training records to the EHD by February 9, 2012. <br />REPEAT VIOLATION <br />605. The used oil tank was observed, located underground, was observed with a label at the drainage/ <br />entry point marked with the words "hazardous waste," but no accumulation start date. All hazardous <br />waste tanks shall be marked with the words "Hazardous Waste" and the accumulation start date. <br />Immediately ensure that all hazardous waste tanks are marked with the required information. <br />REPEAT VIOLATION <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE <br />
