The following is an itemized list of hazardous waste violations that have not been addressed <br />for Ryder Truck Rental #1071 as of June 09, 2015. <br />Open violations from March 18, 2015 inspection <br />Violation #110 - Failed to complete training on hazardous waste management and emergency response <br />procedures. <br />According to Mr. Shannon, the facility has 15 employees. The training records on site only indicated 12 <br />employees attended the annual training. Personnel who handle hazardous waste shall be trained within six <br />months of assignment and take part in an annual review of the initial training received. At a minimum, the training <br />program shall be designed to ensure that facility personnel are able to respond effectively to emergencies by <br />familiarizing them with emergency procedures, emergency equipment, and emergency systems, including where <br />applicable: <br />1. procedures for using, inspecting, repairing, and replacing facility emergency and monitoring equipment; <br />2. key parameters for automatic waste feed cut-off systems; <br />3. communications or alarm systems; <br />4. response to fires or explosions; <br />5. response to ground -water contamination incidents; and <br />6. shutdown of operations. <br />The owner or operator shall maintain the following documents and records at the facility: <br />1. the job title for each position at the facility related to hazardous waste management, and the name of each <br />employee filling each job; <br />2. written job description for each position listed above; <br />3. written description of the type and amount of both introductory and continuing training that will be given to <br />each person filing positions listed above; <br />4. records that document that the training has been given to and completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. Training records on former <br />employees shall be kept for at least three years from the date the employee last worked at the facility. <br />Immediately provide employees with initial or refresher hazardous waste training. Submit a copy of the roster and <br />the syllabus to the EHD. <br />Violation #111 - Failed to maintain complete personnel training records. <br />Employee training records for the management of hazardous waste were incomplete. The records did not include <br />a list of each employee with their job titles. (The job titles on site in the training records were Technician, Service <br />Attendant, Customer Service Coordinator/Service Writer.) Employee training records shall include: <br />1. the job title for each position at the facility related to hazardous waste management, and the name of each <br />employee filling each job; <br />2. written job description for each position listed above; <br />3. written description of the type and amount of both introductory and continuing training that will be given to <br />each person filing positions listed above; <br />4. records that document that the training has been given to and completed by facility personnel. <br />Training records on current employees shall be kept until closure of the facility. Training records on former <br />employees shall be kept for at least three years from the date the employee last worked at the facility. Ensure <br />that employee training is properly documented. <br />Violation #122 - Failed to submit a written response within 30 days of receiving an inspection report. <br />An inspection was last done on January 9, 2014 and an inspection report was issued identifying information to be <br />submitted to bring this site into compliance. This information was required to be submitted by February 9, 2014. <br />This information has not been completely received resulting in a non-compliant status for this facility. The missing <br />information includes a response to the following violations: <br />- CCR 66265.52 Contingency plan incomplete <br />- CCR 66266.130(c)(3) Failed to store used oil filters in a closed, rainproof container that is labeled "drained used <br />oil filters" and marked with the accumulation start date <br />- HSC 25185(c)(3) Failed to submit a written response within 30 days of receiving an inspection report <br />An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br />statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that <br />a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br />inspection report. <br />Page 1 of 3 <br />
