The following is an itemized list of hazardous waste violations that have not been addressed <br />for Ryder Truck Rental #1071 as of June 09, 2015. <br />Open violations from March 18, 2015 inspection <br />Violation #407 - Contaminated container not marked with date emptied or managed within one year of <br />being emptied. <br />Two 55 gallon blue steel drums which previously held oil were observed in the back storage area without empty <br />dates. Containers larger than five gallons that previously held a hazardous material shall be marked with the date <br />they were emptied and managed within one year of being emptied. Immediately mark and manage all empty <br />containers per this section. <br />Violation #605 - Failed to completely label containers or tanks of hazardous waste. <br />A 55 gallon black steel drum of waste absorbent was observed in the shop without a hazardous property on the <br />label. All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties (ignitable, corrosive, reactive, toxic) <br />- physical state (liquid, solid) <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all hazardous waste containers are marked with all the <br />required information. <br />Open violations from January 9, 2014 inspection <br />Violation #110 - Contingency plan incomplete <br />The contingency plan for this facility did not include the descriptions and capabilities of the emergency equipment <br />at the facility, including the cartridge respirators, chemical protective suites, chemical protective gloves, fire <br />extinguishing system, and absorbant. A contingency plan shall include all of the following: <br />-actions facility personnel shall take in response to fires, explosions, and unplanned release to air, <br />soil, or surface water <br />-arrangements agreed to by local police departments, fire departments, hospitals, cleanup <br />contractors, and state & local emergency response teams <br />-names, addresses & phone numbers (office & home) of the Emergency Coordinator and any <br />alternates <br />-list of all emergency equipment at the facility (fire, spill, communications/alarms, <br />decontamination), including location, physical description, and a brief outline of its capabilities <br />-evacuation plan, including at least two routes and signals to be used <br />-telephone number of Cal EMA <br />This information must be kept current at all times. Immediately update the contingency plan for this facility to <br />include all of the necessary information and submit a copy of the plan to the EHD by February 9, 2014. REPEAT <br />VIOLATION <br />Violation #135 - Failed to submit a written response within 30 days of receiving an inspection report <br />The Return to Compliance paperwork for the August 19, 2013 inspection was not submitted. A deficiency letter <br />was issued on December 2, 2013. The operator of the site or facility which receives an inspection report shall <br />submit a written response to the department or the local officer or agency within 60 days of receipt of the <br />inspection report, or with a shorter time as the department or local agency requires. This response shall include a <br />statement documenting the corrective actions that have been or will be taken by the operator. Immediately <br />submit all necessary follow up paperwork for the 2013 hazardous waste generator inspection to the EHD. <br />REPEAT VIOLATION <br />Page 2 of 3 <br />
