(continued) <br /> consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, <br /> and submit copies to the EHD by May 30, 2014. REPEAT VIOLATION <br /> 128. Bills of lading for the recycling of used oil filters for 2011 and 2012 were not found on site. A copy <br /> of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of <br /> lading for used oil filters for the last three years, maintain them on site, and submit copies to the EHD by <br /> May 30, 2014. <br /> 305. Two fire extinguishers in the auto shop were observed with outdated annual maintenance dates, <br /> one dated July 9, 2009 and the other dated July 12, 2012. All facility communications, alarm systems, <br /> fire protection equipment, spill control equipment, and decontamination equipment shall be tested and <br /> maintained as necessary to assure its proper operation in time of emergency. Immediately have all <br /> necessary inspections and service completed for all fire extinguishing equipment at this facility, and <br /> ensure proper maintenance is conducted in the future. <br /> 401. According to Ms. Tingo, one of two science teachers at the school, hazardous wastes generated in <br /> the science classroom labs, such as the acids and bases from the pH lab, are all mixed together into a <br /> single bucket for disposal. The owner or operator shall take precautions to prevent accidental ignition or <br /> reaction, including mixing and commingling, of incompatible wastes and materials. The owner or <br /> operator shall document this compliance.
