The owner of this facility is Interlake Companies, Inc., 550 Warrenville Rd., Lyle,
<br />IL 60532. Interlake does business in California at this location as Lodi Fab
<br />Industries, Inc. The president of Interlake Companies, Inc. is Bob Gruem. The
<br />plant manager for Lodi Fab Industries, Inc. is Dale Dieckbernd. The facility
<br />personnel responsible for hazardous waste management at the 1029 S.
<br />Sacramento St. facility are Peter Eggleton, Human Resources Manager and
<br />Steve Mathias, Industrial Engineer.
<br />Interlake has operated this business since 1971 when they purchased it from
<br />Lodi Fab Industries, Inc.
<br />On March 6, 1981, the State of California Department of Health Services issued
<br />an interim status document number CAD074642901 to Interlake, Inc., 1029 S.
<br />Sacramento St., Lodi, CA. On September 9, 1989, Public Health Services -
<br />Environmental Health Division (PHS-EHD) received an anonymous complaint
<br />(#89-1961) regarding the storage of ten barrels of waste oil behind the machine
<br />shop. According to the complaint, the barrels leak waste oil into the industrial
<br />sewer line when it rains. The complaint was originally reported to Dennis
<br />Fields, San Joaquin County Office Of Emergency Services, who attempted to
<br />get the name of the complainant for further information, but was unsuccessful.
<br />Mr. Fields referred the complaint to PHS-EHD for further investigation.
<br />On September 29, 1989, an inspection was conducted by Elizabeth Ambacher,
<br />Hazardous Materials Specialist, California Department of Health Services; Kasey
<br />Foley, R.E.H.S., PHS-EHD; and Steve Mathias, Industrial Engineer, Interlake
<br />Inc., to investigate complaint #89-1961. The complaint alleging unlawful
<br />storage and disposal of waste oil was not verified during the inspection.
<br />However, as a result of the inspection, violations of the hazardous waste
<br />statutes and/or regulations were identified as follows:
<br />Section 66508 (a)(c), Title 22, California Code of Regulation (CCR):
<br />Drums containing hazardous waste were not labeled "Hazardous Waste".
<br />2. Section 67105 (d), Title 22, CCR: The facility did not have an employee
<br />training program for handling hazardous waste.
<br />On March 2, 1990, a Report of Violations (R.O.V.), and Schedule of Corrections
<br />was drafted by Kasey Foley. On March 21, 1990, the R.O.V. was mailed to Mr.
<br />Steve Mathias, Interlake, Inc., 1029 S. Sacramento St., Lodi, CA 95240.
