5.0 EMERGENCY ALERT PROCEDURES: <br />In the event of an emergency or disaster, the warning may come from any of the following <br />sources: an employee, visitor, commercial radio, television, building fire/smoke detection, fire <br />sprinkler system, emergency siren, web/internet, private citizen, security department or any law <br />enforcement agency. It is recommended that several sources be monitored to assist in determining <br />when emergency situations exist since no one system can cover all circumstances. A person <br />receiving notification of a possible disaster or building emergency shall immediately sound the alarm <br />to notify employees and inform the immediate Supervisor who will continue notification up the chain <br />of command. <br />Below is the general procedure to alert for an emergency: <br />a. Immediately contact a Supervisor to provide a verbal report. <br />b. If unable to find a Supervisor, go to the nearest plant phone and DIAL 85 to page for the <br />Supervisor over the PA system. <br />C. To summon assistance from the FIRE, POLICE OR AMBULANCE: <br />go to the nearest plant phone and DIAL 9 (for outside line) theft DIAL 9-1-1 <br />Provide the dispatcher with the following information: <br />911 <br />• Nature of Emergency ? What is the problem ? <br />• Location of Emergency ? Give address, building number, door number <br />Is Your Name Phone number and the Extension you are calling from <br />Send personnel to wait outside the building to direct incoming responders immediately to <br />where they're needed. <br />d. To summon assistance from Dart Emergency Responders: <br />Step 1: Go to nearest phone - DIAL : 85 (Plant -wide PAGE Number) <br />Step 2: For one of the following emergencies, Speak clearly and slowly: <br />First Aid - Medical Emergency: "Attention First Aid Responders: Report to (state exact location) <br />Hazardous Material Spill: "Attention Hazmat Team: Report to (state exact location) <br />Fire Emergency: "Attention Supervisors: Report to (state exact location) <br />e. In the event the PA System or other alarm system fails, a megaphone is kept in the <br />Production Supervisors Office. <br />• r, <br />7 <br />
