Kennedy/Jenks Consultants <br /> Section 5: Health and Safety Hazard Assessment <br /> 5.1 Potential Hazards <br /> This section discusses the potential physical and chemical hazards associated with the site. To <br /> address these hazards, the SSO will conduct a kick-off safety meeting with all project field staff <br /> before commencing work to review these hazards and mitigation measures. In addition, the <br /> SSO will conduct a daily tailgate safety meeting to discuss the hazards and safety requirements <br /> associated with the work being performed that day. The topic of the safety meetings will be <br /> recorded on the daily safety meeting form; an example is provided in Table 1. The safety <br /> meetings should include contractors working with Kennedy/Jenks field staff so that their work <br /> and safety requirements can be coordinated. Prior to commencing field activities, the Union <br /> Pacific Project Manager will be notified. <br /> 5.2 Vehicle Safety <br /> The following vehicle safety requirements will be followed at all times when working at the site: <br /> 1. Seatbelts must be worn at all times when in moving vehicles. <br /> 2. All construction vehicles must have audible backup warning devices. Devices can be fit <br /> into backup light sockets on vehicles. <br /> 3. Vehicles will have a first-aid kit and fire extinguisher. <br /> 4. Never exceed a 15 miles per hour (mph) speed limit when driving on the site. <br /> 5. Always drive with headlights on when on the site. <br /> 6. Unless authorized by Union Pacific, never park a vehicle or equipment within 250 feet of <br /> a railroad crossing to avoid blocking the view down tracks at crossing. <br /> 7. Unless authorized by Union Pacific, never park a vehicle or equipment within 25 feet of <br /> any track. <br /> 8. Always stop and look both ways at vehicle crossing. <br /> 9. Yield to all trains, trucks, and heavy equipment. <br /> The following additional requirements apply to non-passenger equipment: <br /> 1. Stop the engine and properly secure the equipment against movement whenever not in <br /> use. <br /> 2. Verify that the master battery switch is left in the "off' or"disconnect" position and <br /> padlocked when not in use. <br /> 3.
