Responsible parties not eligible for reimbursement from the Fund may petition the State <br /> 2914 T Straet. Water Resources Control Board for a review of their case. All petitions should be sent to the Fund <br /> Suite 130 Manager. This guidance document has been developed to assist parties in preparing a petition for <br /> Saaatnc to,CA submittal to the Fund Manager. This document specifically delineates what must be contained in the <br /> 16)227-4360 submittal package for closure consideration by the Fund Manager. <br /> FAX(9 16)227-4530 <br /> Beginning January 1, 1997,the tank owner, operator or other responsible party may petition the Fund <br /> woda Wide Web: Manager for a review of their case. It has been the Fund's experience that a minimum of tasks must <br /> l,��r-cwp vw�s.,� have been completed before such requests can be processed. The tank owner, operator or other responsible party believes that their corrective action plan has <br /> been satisfactorily implemented. - <br /> 2. The tank owner, operator or other responsible party has requested from the regulatory agency <br /> that their case be closed. <br /> 3. The regulatory agency has denied the closure request. <br /> 4. The leaking petroleum tank has been removed. . <br /> 5. Any petroleum saturated soil within the tank pit has been removed. <br /> 6. Any free product(f present) has been removed to the maximum extent practicable. <br /> 7. The site has been adequately characterized. <br /> If the tank owner, operator or other responsible party has not completed all of the above items, <br /> the petition will be returned to them. If a tank owner, operator, or other responsible party meets the <br /> requirements above, a package must be submitted which contains all of the following documents: <br /> 1. A one to three page Narrative Summary of the rationale justifying why the case should be <br /> considered for-closure. Basically,this information addresses the items included on the <br /> "Underground Storage Tank Case Review Form" (Attachment 1). As a minimum,the summary <br /> must address the following points: <br /> a. Information on tank removal or repairs and dates . The information (report) must contain <br /> scaled drawings of all appurtenances, sampling locations, excavation limits, etc. <br /> b. If applicable, information on any existing tank(s) in place and dates of installation and <br /> capacities. <br /> c. Information on the extent, removal efforts, and any recharge/recovery rates of free product(f <br /> present). <br /> d. Information on the delineation of contamination (both soil and/or groundwater). Include all <br /> sampling information available,including location, concentrations,dates of sampling, and a <br /> pian view map indicating areal extent of soil and/or groundwater contamination. <br /> e.
