A scaled site reap with legend indicating locations of old and new fuel tanks, fuel lines,soil <br /> borings .groundwater monitoring wells, property boundaries and adjacent streets. <br /> 2. Geologic cross sectional view of site(if available). <br /> 3. Representative potentiometric map that indicates groundwater gradient(s)(flow direction). <br /> g. Rationale or justification for closing the site. <br /> IMPORTANT POINT: A copy of the Narrative Summary must also be sent to the caseworker <br /> assigned to your case with the regulatory agency responsible for oversight of your case <br /> concurrent with your submittal to the Fund Manager. You are not required to send any reports <br /> to the oversight agency, only the Narrative Summary. <br /> 2. A completed "Underground Storage Tank Case Review Form" (Attachment 1). See also example <br /> of completed Form (Attachment 2). <br /> NOTE: SINCE THE FUND MANAGER DOES NOT HAVE ALL THE TECHNICAL REPORTS <br /> PERTAINING TO EACH SITE,YOU MUST INCLUDE ALL APPLICABLE TECHNICAL <br /> REPORTS ALONG WITH YOUR SUBMITTAL PACKAGE. <br /> It must be stressed that complete packages are a necessity for a timely and accurate review. <br /> Incomplete packages will delay the review time since additional requests must be made for more <br /> information. Also, please note that we are looking for concise,thorough and complete packages. <br /> Remember that your package should represent a "stand alone"document that will enable the Fund <br /> Manager to obtain a"big picture" assessment of your site within a short time frame and allow us to <br /> make a timely decision regarding closure of your site. <br /> When a package is received, it will be reviewed by the Fund in close consultation with the regional <br /> board, local regulatory agency or local implementing agency. If all parties agree that the site should <br /> be closed,the site will be recommended for closure and the oversight agency will issue the closure <br /> notice.
