This proposal includes.a scope of work, attached cost estimates, general conditions, and <br /> commercial terms. <br /> SCOPE OF WORK <br /> Work Plan ! <br /> ENSR will prepare a work plan outlining the scope and details of the fieldwork involved with the <br /> well destruction. ENSR will obtain the necessary permits and submit the work plan to the San <br /> Joaquin County, Environmental Health Division (SJCEHD) for approval. Following approval of <br /> the work plan, ENSR will schedule the fieldwork with a drilling contractor and perform the well <br /> destruction activities. <br /> Well Destruction <br /> i <br /> This proposal provides two separate estimates - one estimate to destroy three groundwater- <br /> monitoring wells (MW-1, MW-2 and MW-3) only (Cost A) and another estimate to destroy the <br /> three monitoring wells and a water supply well of unknown depth (Cost B). The estimate to i <br /> include the water supply well is being provided per the request by SJCEHD. The SJCEHD is of <br /> the opinion that destruction of the water supply well would eliminate its potential to act as a <br /> conduit for surface or shallow contaminants to reach the aquifer below. <br /> The monitoring wells will be destroyed using a hollow-stem auger drilling rig by the following <br /> procedures: <br /> • Each well will be sounded to check for obstructions; <br /> • The monitoring well will be over-drilled to remove the filter pack, well casing and annular <br /> seal; <br /> The borehole created will be sealed with neat cement, by tremmie grouting from the <br /> bottom of the borehole to ground surface. <br /> RRecycled PaperlSoy-based Inks <br /> LaENSRIPROPOSALS18728A32.doc <br />
