One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested. <br /> 6 Each individual installing or testing any component shall hold a certificate of <br /> training from the manufacturer of that component. <br /> 7 Complete and submit a copy of a UST Installation Certificate of Compliance <br /> (Former Form C) to this office no later than 30 days after work is completed. <br /> 8 All parts used for this UST Retrofit must be certified by an independent testing <br /> organization and labeled as required by the organization. (UL, ULC, ANSI, API, <br /> ASTM, etc. See California Code of Regulations, Title 23, Chapter 16 Table B for <br /> complete list.). Pursuant to the Underwriters Laboratories Inc (UL) listings, only <br /> those products bearing the UL mark will be considered listed (only applicable to <br /> UL listed products). <br /> 9 Permit expires 180 days from date of approval.
