Montesano <br /> Re: 1711 Yosemite Avenue, Manteca SR0054469 <br /> The permit application to remove all existing piping and under dispenser containment at <br /> the underground storage tank system at the above referenced facility has been approved <br /> with the following conditions: <br /> 1. Contact the fire department and the San Joaquin Valley Air Pollution Control District <br /> for applicable permits. <br /> 2. If the backfill material and/or soil is found to be contaminated it must be disposed of <br /> at an approved disposal location. Treatment of soil onsite can only be performed <br /> under a hazardous waste treatment permit. <br /> 3. See pages 3 of the permit application for required sampling and analytical tests. <br /> 4. A copy of the fire department permit must be submitted at the time of removal. <br /> 5. Schedule removal and sampling appointment 48 hours in advance. <br /> 6. If the pea gravel is contaminated than it cannot be reused and will have to be disposed <br /> according to California Code of Regulations, Title 22 Hazardous Waste Regulations. <br /> 7. Sampling firm shall be an independent third party from the contractor. <br /> 8. In addition, the following information must be submitted within 30 days of the <br /> removal date: <br /> (a) Analytical results,mailed from the lab <br /> (b) Tank closure certification form <br /> (c) Hazardous waste manifest for piping and rinsate <br /> 9. All personnel onsite shall have required hazardous waste training pursuant to title 8. <br /> 10. Permit expires 180 days from date of approval.
