I had <br /> some concerns with it. <br /> Levi didn't leave an email address, so I thought I'd just send them to you. <br /> First..you and I and most ARCO folks know this site to have an address based on <br /> the SJ County Assessor's maps. <br /> It's 2478 E. Oak Street. Levi used the address on the restaurant. This is a post <br /> office address and neither the EHD, the SJ County Assessor and Geotracker <br /> recognize this one. Please ensure that your staff understands this site specific info. <br /> Also, the permit sent in is/was for ALL four MWs included in the work plan (TRAP, <br /> addn). Only MW-5 and MW-8 are located on the site. A permit is only good PER <br /> parcel, per C-57 contractor, and per workplan. MW-6 and MW-7 are located on <br /> other parcels. I issued the permit for ONSITE MW-5 and MW-8. <br /> attached are three maps. <br /> One clearly shows the parcel where MW-6 is proposed. Use this site's address and <br /> provide an access agreement signed by this person when you submit another EHD <br /> well permit application and $89 fee. <br /> the other two maps were attached since I'm NOT sure where you plan on putting <br /> MW-7. As you can see, the parcel line goes thru the parking lot mostly owned by <br /> one entity and the restaurant lot owned by yet ANOTHER entity. After you decide <br /> exactly which parcel MW-7 will be on, send in another application, $89 fee and a <br /> signed access agreement by this land owner. <br /> address on the EHD Well Permit Application form should be the address of the <br /> parcel noted on these maps.
