Hwy. 132 <br /> Vernalis, CA 95385 <br /> On April 15, 1999 San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) received the "Preliminary Site Assessment Work Plan„ dated April <br /> 8, 1999 that had been prepared by Advanced GeoEnvironmental Inc. (AGE) for the <br /> above referenced site. <br /> As the responsible party for the site you were notified by PHSfEHD in correspondence <br /> dated June 16, 1999 that the work plan was approved with the modification that the <br /> lateral assessment of the contamination be extended to cover the south side of the <br /> former tank pit area.
