Revocable Permit No. 03-19W <br /> Advanced Geoenvironmental Incorporated <br /> Page 2 <br /> 4, In accordance with Section 1(h) of the "Standard Conditions Applicable to <br /> Revocable Permits,"the permittee shall provide and keep in force during the term of <br /> the permit a policy of public liability insurance against liability for injuries to persons <br /> or property arising out of accidents attributable to the structure on City property, in <br /> amounts to be determined by the Risk Manager. The policy of insurance so <br /> provided shall contain a contractual liability endorsement covering the Ilabllity <br /> assumed by the Permittee by the terms of his permit and shall contain a provision <br /> that such policy may not be canceled except after thirty (30)days notice in writing to <br /> be given to the Risk Manager. The copies of these policies or certificates <br /> evidencing the same shall be filed with the Risk Manager of the City after approval <br /> of the City Attorney. <br /> 5. The issuance of this permit by, the City does not create any agency on the part of <br /> the permittee, and the permittee will hold the City of Stockton harmless for any <br /> action for damages arlsing odt of the permittees activities performed under this <br /> permit, <br /> 6. Permittee, or his contractor,shall obtain an Encroachment Permit for the installation <br /> of the well and any future mai�IStenance of the well. <br /> 7. The well head shall be coveretf with a meter box or valve box as approved by the <br /> City of Stockton. The top of t le box shall be flush with the surrounding ground or <br /> pavement. <br /> i <br /> 8. The provisions of the State of 6alifornia, Department of Water Resources Sulletln <br /> 74-81 and 74-90 entitled "Water Well Standards: State of Caiifomia" shall be <br /> applicable to this permit. <br /> 10. Construction operations that Jeate loud noise shall be confined between the hours <br /> of 7 a.m. and 6 p.m. <br /> 12. Disposal of water from the well development shali be in accordance with current <br /> federal regulations. <br /> 13. The barricading of any portion bf City streets shall be subject to the approval of the <br /> City Traffic Engineer. Traffic crines and warning signs shall be used to d irect traffic <br /> and shall be installed in accordiance with the Caltrans Manual of Traffic Controls for <br /> Construction and Maintenances Work Zones or the WATCH Manual, <br /> i <br /> i <br />
