Revocable Permit No. 03-19W Advanced Geoenvironmental Incorporated Page 2 4, In accordance with Section 1(h) of the "Standard Conditions Applicable to Revocable Permits,"the permittee shall provide and keep in force during the term of the permit a policy of public liability insurance against liability for injuries to persons or property arising out of accidents attributable to the structure on City property, in amounts to be determined by the Risk Manager. The policy of insurance so provided shall contain a contractual liability endorsement covering the Ilabllity assumed by the Permittee by the terms of his permit and shall contain a provision that such policy may not be canceled except after thirty (30)days notice in writing to be given to the Risk Manager. The copies of these policies or certificates evidencing the same shall be filed with the Risk Manager of the City after approval of the City Attorney. 5. The issuance of this permit by, the City does not create any agency on the part of the permittee, and the permittee will hold the City of Stockton harmless for any action for damages arlsing odt of the permittees activities performed under this permit, 6. Permittee, or his contractor,shall obtain an Encroachment Permit for the installation of the well and any future mai�IStenance of the well. 7. The well head shall be coveretf with a meter box or valve box as approved by the City of Stockton. The top of t le box shall be flush with the surrounding ground or pavement. i 8. The provisions of the State of 6alifornia, Department of Water Resources Sulletln 74-81 and 74-90 entitled "Water Well Standards: State of Caiifomia" shall be applicable to this permit. 10. Construction operations that Jeate loud noise shall be confined between the hours of 7 a.m. and 6 p.m. 12. Disposal of water from the well development shali be in accordance with current federal regulations. 13. The barricading of any portion bf City streets shall be subject to the approval of the City Traffic Engineer. Traffic crines and warning signs shall be used to d irect traffic and shall be installed in accordiance with the Caltrans Manual of Traffic Controls for Construction and Maintenances Work Zones or the WATCH Manual, i i
