The following alarms were not entered <br /> into the log book and addressed by the facility: <br /> S11:91 Product Communication Alarm on 2/14/16 and 2/27/16; <br /> S7:87 Product Vacuum Communication Alarm on 2/23/16; <br /> S11:91 Product Vacuum Warning and Communication and Sensor Fault Alarms on 7/26/16; <br /> Q2: Premium Gross Line Fail and PLLD Shutdown Alarm on 8/21/16; <br /> S11:91 Product Communication Alarm on 9/15/16; <br /> Unleaded Regular Sudden Loss Alarm on 10/28/16; <br /> Q2: Premium Gross Line Fail and PLLD Shutdown Alarm on 10/21/16; <br /> S10: Vapor Lines Communication Alarm on 11/19/16; and <br /> S12:91 Vent Communication Alarm on 12/19/16. <br /> All alarm records shall be maintained on site for at least three years. Monitoring records include: (1) date and time <br /> of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any <br /> visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges <br /> or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br /> reconciliations. Immediately implement a program for recording alarms in the daily alarm log and note what action <br /> was taken for each alarm. Maintain all maintenance, monitoring records and alarm logs on site going forth. <br /> This is a minor violation. <br /> 304 HSC 25290.1(d) Monitoring system not maintained to detect entry into secondary containment(after Jul 2004). <br /> The L8 87-product fill sump sensor 208 failed when tested. The continuous monitoring system shall be maintained <br /> to detect the entry of the liquid or vapor phase of the hazardous substance stored in the primary containment into <br /> the secondary containment and to detect water intrusion into the secondary containment. The sensor was replaced, <br /> like for like, and retested during the inspection. A"UST Retrofit Verification with Inspector Already Onsite" form has <br /> been completed and provided to the operator and contractor. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 305 HSC 25290.1(e) VPH monitoring of the interstitial spaces of the UST system is not maintained. <br /> Communication in the 87-, 91-, and diesel- product submersible turbine pump (STP) sumps and fill sumps, and <br /> communication in the under dispenser containment(UDC) sumps were not tested during the annual monitoring <br /> system certification testing on February 1, 2017. The interstitial space of the underground storage tank system shall <br /> be maintained as designed under constant vacuum such that a breach in the primary or secondary containment is <br /> detected before the hazardous substance is released into the environment.
