k'q KLEINFELDER <br /> Additionally, PHS/EHD will bill Mark Thomas & Company for oversight fees in addition to the <br /> permit fee to be submitted by Kleinfelder. These fees will be billed directly to Mark Thomas & <br /> Company by the PHS/EHD, with a minimum charge of 3 hours ($234). Kleinfelder will forward <br /> the appropriate "billing and compliance acknowledgement forms" to Mark Thomas & Company <br /> for signature. Signed originals of these forms, along with a check for the initial 3 hour minimum <br /> charge, must be submitted to PHS/EHD as part of the permit process. <br /> Prior to initiating field activities, Kleinfelder will prepare a site specific health & safety plan. <br /> Kleinfelder will also contact Underground Service Alert 48 hours prior to conducting field work <br /> to notify local utilities of our proposed subsurface assessment. <br /> Task 2.2—Soil Sample Collection <br /> Kleinfelder proposes to collect soil samples from exploratory borings advanced using a truck <br /> mounted drill rig equipped with hollow stem auger. Soil samples will be collected at a depth of <br /> 5 feet, and at 5 foot intervals to a maximum depth of 20 feet. Groundwater is anticipated to exist <br /> at an approximate depth of 60 feet below existing grade and groundwater sampling or deeper soil <br /> sampling is not included in this scope of work. The soil samples collected will be field screened <br /> using an organic vapor detector as described in Attachment B, Section B-2.2. The borings will <br /> be drilled, logged, and sampled following the protocol described in Attachment B, Section B-2.3. <br /> Each exploratory soil boring will be grouted the same day in accordance with PHS/EHD <br /> requirements. <br /> Task 2.3—Laboratory Analysis <br /> Each of the soil samples selected for quantitative analysis will be submitted under chain-of- <br /> custody control to a laboratory certified by the State of California for the requested analyses. <br /> Samples will be analyzed on normal laboratory turnaround schedule, requiring approximately 10 <br /> working days for receipt of analytical data.
