The previous owner of the <br /> property has advised PHS-EHD that you are the current owner. <br /> This site is currently assigned to the Corrective Action Program (CAP) of the Site Mitigation Unit of <br /> PHS-EHD and is required to investigate the extent of soil and groundwater contamination known to <br /> be documented at this site. <br /> Previous subsurface investigations have not yet determined the lateral extent of either the soil or <br /> groundwater plume of contamination and as such, further work is required at the site. <br /> As the new owner and responsible party, you are required to continue to investigate the <br /> unauthorized release from the leaking underground fuel tank. Please submit a workplan by <br /> December 16, 1994 that will address the next phase of investigation. <br /> Please complete and sign the enclosed forms and send them along with your workplan. Sites within <br /> CAP are to submit a five (5) hour prepayment of $390.00 for review and oversight prior to start up <br /> of their investigation or remediation.
