The Fifth Amended Permit incorporates the change requested in your <br /> letter of March 19, 1993. 1 am also enclosing a black lined copy of this Fifth Amended <br /> Perniit to show the changes that were made since inception. <br /> 1. The wells and test borings shall be advanced using conventional hollow stem <br /> auger equipment. <br /> 2. The monitoring wells shall be installed at a depth no greater than 20 feet. <br /> Borings shall be drilled to,depths no greater than 15 feet providing the casings are no greater <br /> than 6" diameter. Monitoring wells shall be grouted with neat cement. <br /> 3. Regarding the Monitoring Wells, selected soil borings will be converted to <br /> two-inch diameter ground water monitoring wells. These wells will be used to assess the <br /> ground water flow.direction and to provide an initial assessment of the potential extent of <br /> petroleum hydrocarbon contaminants in ground water. The wells will be constructed in <br /> accordance with Section 2647 of the California Administrative Code(CAC, 1985) and-the <br /> Department of Water Resources'Bulletin 74-81 (DWR, 1981). Permittees will obtain a <br /> written permit from the District prior to construction of any wells. <br /> i <br /> The wells will be screened from a total depth of 15 feet to 5 feet below the <br /> ground surface (10-foot screened intervals). The wells will be constructed of Schedule 40 <br /> PVC blank casing and 0.20-inch machine slotted screen. A clean silica sandpack will be <br /> placed around the well screen. A two foot bentonite seal will be placed above the sandpack. <br /> The remaining annulus will be filled with bentonite/cement grout. A watertight locking cap ' <br /> will be installed on the well and the wellhead will be protected by installing an at-grade, <br /> watertight, traffic rated road box. <br /> L <br /> Each monitoring well will be checked for the presence of free product. If free <br /> product is present, its thickness will be measured and the free product will be hand bailed <br /> from the wells prior to development. <br /> The wells will be developed by pumping and surging until the clarity of the <br /> purged ground water stabilizes.
