NEW WELL PERMIT REQUIREMENTS <br /> Department of Health Services <br /> Division of Drinking Water <br /> 31 East Channel St. # 270 <br /> Stockton, CA 95202 <br /> ❑ 1. Permit Application — Permit application form (attached) for addition of the new <br /> drinking water source(s) must be submitted to the Drinking Water Field _ <br /> Operations Branch (DWFOB), as required by Section 116550, Chapter 7, Part 1, <br /> Division 5, of the California Health and Safety Code. <br /> ❑. 2. Permit Application Processing Fee -- (Only_applicable for systems serving less <br /> than 1000 service connections}. Payable to "Department of Health Services". <br /> Payment of the appropriate fee ($253.00 for an amendment due to addition of a <br /> source) must accompany the application for the permit or permit amendment. <br /> ❑ 3. Map(s) showing the location of the well/proposed site - with respect to other <br /> water system facilities. <br /> ❑ 4. Drinking Water Source Assessment and Protection Program (DWSAP) — <br /> Many activities that occur in communities have the potential to impact the water <br /> that is pumped from a community wells. The DWSAP is an expansion of the <br /> older Federal wellhead protection program and is the first step in the <br /> development of a complete source water protection program. The DWSAP <br /> includes delineation of the area around a drinking water source through which <br /> contaminants might move and reach the source and identification of <br /> contaminants and their sources within the delineated area in order to assess the <br /> susceptibility of the water source to these contaminants. <br /> As part of the permit application, public water systems are now required to <br /> submit a source assessment in accordance with the DWSAP. The source <br /> assessment will be considered in the permitting of the new wells. A <br /> complete source assessment should include all the necessary elements (e.g. <br /> delineation of source area and protection zones, physical barrier effectiveness, <br /> inventory of possible contaminating activities around the well, report summary <br /> etc) of DWSAP in the report. <br /> Note that DWSAP guidance and other related information including all required <br /> forms is already available on the Department's Internet web page.
