11.01-1110- _ - "Alk - .. s <br /> • Time and type of incident(e.g., release, fire); <br /> • Name and quantity of material(s) involved; <br /> • Extent of injuries, if any; and <br /> Possible hazards to human health, or the environment. <br /> If an evacuation of the surrounding areas is necessary,the emergency coordinator <br /> will immediately report the release to the Stockton Fire Department. The Stockton Fire <br /> Department will notify nearby businesses and facilities as warranted by the emergency <br /> situation. <br /> Any personnel injured during the emergency will be taken to the following <br /> medical facility for treatment: <br /> San Joaquin General Hospital <br /> 1601 E Hazelton Ave <br /> Stockton, California <br /> Within 15 days of the release, Maxim Crane Works will submit a written report <br /> on the incident to the California Office of Emergency Services and the San Joaquin <br /> County Environmental Health Department. The report will include the following: <br /> • Name, address, and telephone number of the Maxim Crane Works facility; <br /> • Date, time and type of incident(e.g., release, fire, explosion); <br /> • Name and quantity of material(s) involved; <br /> • Extent of injuries, if any; <br /> • An assessment of actual or potential hazards to human health or the <br /> environment, where this is applicable; and <br /> B-7 <br />
