Ile 13.0 RECORD KEEPING AND DATA MANAGEMENT <br /> Proper record keeping and data management are,essential in the implementation of this SHSP <br /> The forms associated with the record keeping and data management requirements must be <br /> completed in an accurate, timely fashion and filed with the appropriate entities It 1s the <br /> responsibility of the SS to ensure that the forms are properly completed Completed forms will <br /> be kept and maintained by IT These records will be maintained for a five-year period <br /> Subcontractors will also be responsible for keeping a copy of the forms pertaining to their <br />' personnel <br />' 13.1 togs <br /> The SHSO will maintain and complete a daily log for each day's work The daily log will <br /> document chronologically each day's health and safety activities in sufficient detail for future <br />' reference as needed Other relevant data and field information will be recorded on separate log <br /> forms for air monitoring, sampling, equipment calibration inspections, and incident reporting <br />' An EZ sign-m log will be maintained that will provide a project record of the following <br /> Ile information for confined space entry activities <br /> • worker's name <br /> • work area <br /> • duties performed <br /> • level of protection <br />' time in/time out <br />' All personnel will be required to log in and out of the EZ <br /> A visitors sign-in log will be maintained in the project office and administration area Visitors <br />' requesting access to hazardous field activities must have appropriate project approval, be <br /> medically qualified, and have the health and safety training prerequisites for hazardous waste <br /> roperations <br /> An OSHA 2203 Job Safety and Health Protection poster will be clearly displayed in the site <br />' administration trailer <br /> r <br /> r <br /> c +e�,�e� „ ana�e�uwe�u sn�r„ s�sP doc 1 3-1 <br /> 112402 <br />
