0 <br /> 13.2 Safety Inspections <br /> • IT's accident prevention program is centered around the following key procedures <br /> 1 <br /> Project reporting, investigation, and review of all near misses, incidents, and accidents <br />' • Management reviews of all incident/accident reports, corrective action, and project safety <br /> concerns <br /> • Review of project, operations, and construction activities by health and safety professionals <br /> Safety reviews and inspections are conducted by all tiers of the management structure and are <br /> documented A list of all corrective action items is required to be maintained showing the <br /> corrective action, responsible person, and the date action is to be completed Follow-up <br />' inspections are conducted by health and safety personnel to ensure that corrective actions or <br /> measures have been implemented <br />' The SS will inspect the site weekly and interview one or two site workers regarding areas of <br /> safety concerns or ideas for safety improvement Site supervisory personnel will inspect site <br />' conditions and activities daily to identify changing conditions or potential hazards Identified <br /> safety and occupational health deficiencies and suggested corrective measures will be brought to <br /> the attention of the SS and SHSO Safety review inspections will be recorded and filed for <br /> reference by project management and Waste Management Representative personnel <br />' 13.3 Accident Reporting and Investigation <br /> All project personnel are required to report all near misses, injuries, illnesses, and accidents to <br />' their immediate supervisor The SHSO will immediately arrange appropriate medical care as <br /> required Once immediate medical care for the injured personnel has been accomplished, the <br /> SHSO will complete and submit the appropriate report forms within 24 hours The appropriate <br /> form(s)to be completed may include <br /> IT Supervisor's Employee Injury Report <br /> IT Vehicle Accident Report <br /> IT General Liability , Property Damage, and Loss Report <br /> Copies of the IT forms listed in this SHSP have been sent to the SHSO in a separate Postings, <br />' Permits and Forms package <br /> loIdentified safety and occupational health deficiencies and corrective measures will be <br /> documented and filed on site for reference by the Waste Management Representative <br /> r <br /> C n mb igbwr mmiaAPffieer ruhnwst_$Hsa dw 13-2 <br /> 7/24Uz <br />
