• Summary of onsite meetings <br /> • Deviations from sampling plans and health and safety plan <br /> • Changes in personnel and reasons for the changes <br /> • Levels of personal protective equipment <br /> • Calibration data for field instruments <br /> The field daily reports will include other forms pertinent to the project such as boring logs,well <br /> construction data, chain-of-custody forms, tailgate safety meeting forms, and sample shipping <br /> documentation. <br /> Sample lithology, pertinent drilling and VOC monitoring information will be described and <br /> recorded on field boring logs by an experienced environmental geologist working under the <br /> supervision of a California Certified Engineering Geologist. Lithology and other pertinent data <br /> will be recorded on a field-boring log in accordance with Method D 2488-84 for visual description <br /> and identification of soils. All sampling equipment will be cleaned in an Alconox solution and <br /> double rinsed prior to each use. <br /> Chain-of-custody forms will be maintained to document sample collection and shipment to the <br /> laboratory for analysis. Each sample shipment will be accompanied by a chain-of-custody form to <br /> maintain custodial record of the shipment. The chain-of-custody record will be used to record <br /> sample designation, collection date and time, and requested laboratory analyses. A sample <br /> shipment will be considered to be in one's custody if it is in physical possession, in someone's <br /> view, locked, or kept in a secured restricted area. <br /> Groundwater samples will be collected in 40-milliter VOA vials and will be immediately place in <br /> an iced-chilled cooler maintained at 4 degrees Celsius. The cooler will be filled with packing <br /> material to protect the sample containers from breakage during shipment. The chain-of-custody <br /> form will be placed in a sealed plastic bag for protection. Samples will be shipped to the laboratory <br /> on the collection day or the following day for two-day groundwater sampling events. If samples <br /> are temporarily stored overnight by the sampling team, then the sample cooler will be stocked with <br /> Environmental/LNC/Workplan/Appendix 2.wpd 12 <br />
