6.4 Sample Custody <br /> For each sample that is submitted to the laboratory for analysis, an entry will be made on a chain- <br /> of-custody form supplied by the laboratory. The information to be recorded includes the <br /> sampling date and time, sample identification number, matrix type, requested analyses and <br /> methods, preservatives, and the sampler's name. Sampling team members will maintain custody <br /> of the samples until they are relinquished to laboratory personnel or a professional courier <br /> service. The chain-of-custody form will accompany the samples from time of collection until <br /> received by the laboratory. Each party in possession of the samples (except the professional <br /> courier service)will sign the chain-of-custody form signifying receipt. The chain-of-custody <br /> form will be placed in plastic bag and shipped in the cooler with the associated samples., After <br /> the samples, ice, and chain-of-custody forms are packed in the cooler, the cooler will be <br /> appropriately sealed before it is relinquished to the courier. The laboratory will provide a copy of <br /> the original completed form along with the report of the results. Upon receipt, the laboratory will <br /> inspect the condition of the sample containers and report the information on a chain-of-custody <br /> or similar form. <br /> Environmental/LNC/workplan/Appendix3.wpd 17 <br />
