Appendix S <br /> Health and Safety Plan <br /> Lustre-Cal Nameplate Corp. <br /> 4.0 PRIMARY RESPONSIBILITIES <br /> 4.1 PROJECT MANAGER <br /> The Project Manager(PM) shall: <br /> 1. Direct all Consultant and Contractor personnel involved in investigative,monitoring, and <br /> remedial activities at the Site and vicinity; <br /> 2. Make the Project Health and Safety Officer aware of all pertinent project developments and <br /> plans; <br /> 3. Make available the resources that are necessary for a safe working environment; and <br /> 4. Maintain communications with the client, as necessary. <br /> 4.2 PROJECT HEALTH AND SAFETY OFFICER <br /> The Project Health and Safety Officer(PHSO) shall: <br /> 1. Direct all health and safety aspects of investigative,monitoring, and remedial activities <br /> conducted by Consultant and Contractor personnel at the Site and vicinity; <br /> 2. Ensure that all Consultant personnel have received required training, are aware of the <br /> potential hazards associated with Site operations,have been instructed in the work practices <br /> necessary for personal health and safety, and are familiar with the HASP procedures for all <br /> scheduled activities and for dealing with emergencies; <br /> 3. Direct required exposure monitoring to assess Site health and safety conditions; <br /> 4. Prepare any accident/incident reports; <br /> 5. Modify the Site HASP as required based on accidents/incidents and findings regarding <br /> personnel exposures and work practices; and <br /> 6. Report all accidents/incidents and findings regarding personnel exposure and work practices <br /> to the Project Manager. <br /> 4.3 SITE SAFETY OFFICER <br /> The Site Safety Officer(SSO) shall: <br /> 1. Ensure that appropriate personal protective equipment is available for Consultant and <br /> Contractor site personnel and enforce proper utilization of personal protective equipment by all <br /> on-Site Consultant and Contractor personnel; <br /> Page 5 of 23 <br />
