Appendix 5 <br /> Health and Safety Plan <br /> Lustre-Cal Nameplate Corp. <br /> 2. With guidance from the PHSO, observe subcontractor's procedures with respect to health <br /> and safety. If the SSO believes that a subcontractor's personnel are or may be exposed to <br /> an immediate health hazard,the SSO shall suspend the subcontractor's Site work. If the <br /> subcontractor's personnel do not have required protective equipment,the SSO shall consult <br /> with the PM or PHSO before proceeding with the work; <br /> 3. Implement the Site HASP and report any observed deviations from Site conditions <br /> anticipated in the plan; <br /> 4. Conduct Site safety briefings as needed; <br /> 5. Calibrate monitoring equipment daily and properly record and file results; <br /> 6. Under direction of the PHSO perform required exposure monitoring; <br /> 7. Maintain monitoring equipment or arrange maintenance as necessary; <br /> 8. Assume other duties as directed by the PM or PHSO; and <br /> 9. Report observed accidents/incidents or inadequate work practices to the PHSO and the PM. <br /> 4.4 PROJECT PERSONNEL <br /> Project personnel involved in on-Site investigations and operations shall: <br /> 1. Take reasonable precautions to prevent injury to themselves and to their fellow employees; <br /> 2. Conduct only those tasks that they can do safely and immediately report accidents and/or <br /> unsafe conditions to the SSO or PHSO; <br /> 3. Follow the procedures set forth in the Site Health and Safety Plan and report to the SSO or <br /> PHSO any observed deviations from the procedures described in the plan on the part of <br /> Consultant and Contractor personnel; and <br /> 4. Inform the PM and PHSO of any physical conditions that might affect their ability to <br /> perform the planned field tasks. <br /> 4.5 TRAINING REQUIREMENTS <br /> All project personnel must comply with OSHA regulations specified in 29 CFR 1910.120 and CCR Title <br /> 8, Section 5192. These include completion of a 40-hour health and safety training course, an annual 8- <br /> hour refresher training, and participation in Consultants' medical surveillance program and <br /> respiratory protection program. <br /> Additional Site-specific training that covers on-Site hazards, personal protection requirements, <br /> decontamination procedures, and emergency response information as outlined in this Site HASP will be <br /> given by the PHSO or SSO before beginning on-Site work. Site-specific training briefings will be <br /> documented on a "Project Health and Safety Field Meeting Form" provided at the end of this plan. We <br /> Page 6 of 23 <br />
