Appendix S <br /> Health and Safety Plan <br /> Lustre-Cal Nameplate Corp. <br /> do not anticipate that field staff will be occupationally exposed to blood or potentially infectious materials <br /> during the course of this project. <br /> 4.6 MEDICAL SURVEILLANCE <br /> All Consultant project Site personnel shall participate in the Consultant medical surveillance program, <br /> which includes annual audiometric and physical examinations for employees involved in hazardous waste <br /> or materials projects. It requires that all such personnel have medical clearance before being issued a <br /> respirator and participating in field activities. Frequency of medical examinations which complies with <br /> 29 CFR§ 1910.120(f3)and CCR 8 § 5192(f3), occurs: <br /> 1. Prior to performing field work; <br /> 2. At least once every 12 months; <br /> 3. At termination of employment; <br /> 4. Upon occurrence of possible overexposure; <br /> 5.
