' Secuon 3 <br /> ONSUE Environmental Laboratories Inc Revision 0 <br /> Quality Assurance Plan January 15, 1996 <br /> Page 3 of 5 <br /> Upon receipt in the laboratory, Laboratory personnel make sure that as much <br /> ' information about the samples are recorded on the COC form pnor to signing the <br /> samples into ONSITE custody The ONSITE representative confirms that sample <br /> labels and the COC agree, signs in the area entitled "Received By" and issues a <br /> laboratory identificanon number which is unique to each office, project and sample <br /> 3 1 3 Analysis Requests <br /> ' The record is to be completed in the field to indicate project requested analyses, <br /> sample names and locations The primary documenting record is the COC <br /> ' Although changes from samplmg plan and the analyses planned may be made by <br /> field personnel because of sampling condinons, the analyses requested should be <br /> consistent with the project work plan or Quality Assurance Project Plan(QAPjP) <br /> ' and the reasons for any changes logged m the field notes If, in the event these <br /> documents do conflict, the laboratory will attempt to contact the client prior to <br /> proceeding with the analysis If the conflict is not resolved before the test trust be <br /> ' performed (e g , because holding tunes will expire), the signed COC takes <br /> precedence over other documents <br /> 32 Sample Containers <br /> ONSITE has adopted the sample preservation, container, and holding time <br /> recommendanons published in the US EPA "Test Methods for Evaluating Solid Wastes, <br /> ' Physical/Chemical Methods," (SW-846) and "Guidelines for Establishing Test Procedures <br /> for the Analysis of Pollutants Under the Clean Water Act," (40 CFR Part 136, October, <br /> ' 1984) A surtunary of these items is listed in Appendix B The laboratory will provide <br /> sample containers with appropnate preservatives to clients upon request Shipping <br /> conminers and chain-of-custody forms are provided as part of the sample kit <br /> ' ONSITE does not reuse sample containers All sample containers are used for a single <br /> sampling event, then disposed of m accordance ONSFFEs sample disposal plan The <br /> laboratory uses only vendor provided equivalent of EPA 200 or 300 senes precleaned <br /> rglassware and polyethylene <br /> ' 33 Sample Tracking <br /> 33 1 Sample Identification <br /> ' Samples received at the laboratory are considered to be physical evidence and are <br /> handled according to procedural safeguards established by the EPA Each sample <br /> is given a unique laboratory log number, which is logged into the appropnate <br /> ' bound sample receiving notebook The log number is clearly marked on the <br /> sample container and also recorded onto the COC for future reference The <br /> Word 6 0 loetuc3 doc <br />
