3.3 SUPPLY WELL SURVEY <br /> As requested b the PHS/EHD a survey of domestic municipal, a cultural industrial and other ' <br /> ixl Y Y � , <br /> wells within a 2,000-foot radius of the site will be conducted The survey will include a review of ' <br /> records on file with the Department of Water Resources, California Water Service (the local <br /> murucipal water supplier), the Port, and nearby industries In addition, a physical search of the area <br /> will be conducted to visually identify supply wells in the area , <br /> Locations of all wells identified within the 2,000-foot radius will be depicted on an appropriately <br /> scaled map Construction details of wells located will be supplied when available Based on the ' <br /> information collected during the survey, the influence of supply-well pumping on contaminant <br /> migration will be evaluated as well as identifying and evaluating public or environmental health ' <br /> threats or impacts from contaminant migration <br /> 3.4 TECHNICAL REPORT , <br /> Upon completion of the monitoring well installation, sampling, laboratory testing, and supply well ' <br /> survey, a technical report will be prepared documenting the field activities and summarizing results <br /> and findings of the monitoring well sampling and well survey This report will include figures <br /> depicting the monitoring well locations and existing site features, groundwater gradient, and supply <br /> well locations It will also include tabulated analytical results, well construction logs, and copies <br /> of perrmts obtained for the work ' <br /> In addition, quarterly data reports will be prepared, if required, following subsequent quarterly <br /> sampling of the monitoring wells These reports will summanze field activities and present data ' <br /> in tabular and graphic forms as appropriate <br /> 3.5 HEALTH AND SAFETY <br /> Thisproject's Health and Safety plan for the subsurface investigation is provided as Appendix A ' <br /> A copy of this plan will be available on-site during the field work This plan is not untended for use ' <br /> by other contractor's personnel involved with this work <br /> &3,WMV vvn 8 PHILIP P.
