PHS-EHD issues well installation and destruction permits in <br /> compliance with our county ordinance once the work has been approved by CVRWQCB. PHS-EHD <br /> charges hourly fees for field inspections and office work related to issuing permits. The new property <br /> owner and current facility information must be updated before the permits can be issued. <br /> Enclosed is a Master File Record (MFR)form that is to be completed, signed by the new billing party, and <br /> returned to PHS-EHD. A payment of$267 that will cover three hours of EHD staff time ($89/hour) must <br /> be submitted with the MFR. Staff time over the three hours will be billed to the party listed on the MFR. <br /> Once the MFR and payment are received, the permits will be issued for the monitoring well destructions <br /> and replacement/installations. Staff will notify you so that an appointment to start the fieldwork can be <br /> made. <br /> The permit applications to destroy the production wells (PW-2 & 5)have been given to the EHD well <br /> program unit. Please call 468-3423 to verify the EHD inspector responsible for this area and make an <br /> appointment. <br /> Please submit your completed MFR and $267 for the monitoring well work to Michael Infurna at the <br /> address noted on the letterhead.
