Project No.4-419-0381 <br /> August 29,2019 <br /> Page No.2 <br /> 5192. SALEM employees' certificates of completion for the OSHA 40 hour course will be kept in the <br /> project manager's project file. Subcontractor personnel will be requested to document successful <br /> completion of 29 CFR 1910.120 training or satisfactory equivalent. <br /> The responsibilities of each personnel group are listed below. In general, it is the responsibility of the <br /> Project Manager to ensure that the field team has access to this plan and reads the safety procedures. It is <br /> the individual's responsibility to bring to the attention of the Project Manager any portion of this plan and <br /> related training that he/she does not fully understand. <br /> Prior to beginning site work, there will be a meeting of the field team personnel at the site to discuss the <br /> contents of this plan. The objective of this meeting is to ensure that all field team personnel are adequately <br /> informed in hazard evaluation and safe work practices.The briefing will include as a minimum,discussion <br /> on restricted areas, the nature of potential chemicals at the site, donning personal protective equipment, <br /> decontamination procedures, and emergency procedures. This briefing will occur on-site. Each meeting <br /> will be documented by the Site Safety Officer or his/her designee. Copies will be kept within the project <br /> file. <br /> 1.2.1 SALEM Project Administrator <br /> The health and safety responsibilities of the Project Administrator are to: <br /> • Coordinate the start-up health and safety activities of all site personnel; <br /> • Keep up-to-date information on local, state, and federal health and safety regulations; <br /> • Coordinate health and safety inspection procedures, and follow up to make sure any necessary <br /> corrective action is completed; <br /> • Review final accident reporting and investigation findings; and <br /> • Provide technical liaison to regulatory agency personnel on matters relative to occupational health <br /> and safety. <br /> The Project Manager will report project status to the Project Administrator on a regular basis. The Project <br /> Administrator will immediately notify the Project Manager of any changes in health and safety regulations <br /> or procedures. The Project Manager will immediately notify the Project Administrator if any accidents, or <br /> near accidents occur, regardless of whether personnel were injured. The Project Administrator (Richard <br /> Opp)can be reached at(909)980-6455. <br /> 1.2.2 SALEM Project Manager and Site Health and Safety Officer <br /> The responsibilities of the Project Manager with respect to safety are: <br /> • Ensure that daily site safety meetings are conducted; <br /> • Conduct accident investigations; <br /> • Verify that all on-site personnel have read and understand the Health and Safety Plan; and <br /> • Make available to field team personnel the data known to him/her on the chemical contaminants in <br /> the soils and/or groundwater(if present). <br /> The Project Manager(Shannon Lodge)can be reached at(559)271-9700. <br /> 1.2.4 SALEM Corporation Program Health and Safety Officer <br /> Responsibilities of the Program Health and Safety Officer(PHSO)will include: <br /> • Reviewing and approving the project health and safety plan; <br /> • Provide health and safety information and assistance to the Site Safety Officer, Project Manager, <br /> and field personnel as necessary; <br /> • Assist in conducting accident investigations; and <br /> SALEM <br /> engineering group. inc. <br />
