Project No.4-419-0381 <br /> August 29,2019 <br /> Page No.3 <br /> • Update and change the health and safety plan if warranted by changed conditions and act as the <br /> agency liaison on matters relating to safety and health. <br /> The Program Health and Safety Officer is Shannon Lodge,he can be reached at(559)271-9700. <br /> 1.2.4 SALEM Site Safety Officer <br /> Responsibilities of the Site Safety Officer(SSO)will include: <br /> • Conduct initial site safety meeting; <br /> • Conduct daily site safety briefings; <br /> • Assist in conducting accident investigations; <br /> • Distribute and collect documentation that all personnel have read and understand the Health and <br /> Safety Plan; <br /> • Supervise personnel monitoring procedures; <br /> • Supervise and ensure correct operation and maintenance of personnel monitoring/protective <br /> equipment; <br /> • Supervise and ensure integrity of work zones,including establishment of exclusion,contamination <br /> reduction, and support zones; <br /> • Supervise maintenance and adherence to safe work practices; and <br /> • Ensure proper maintenance of field documentation including calibration logs, site personnel <br /> (entrance/exit)logs, and posting of applicable Federal and State safety placards. <br /> The SSO may directly contact the Project Administrator in the event immediate and threatening safety <br /> hazards,accidents,and/or in the event that immediate corrective action measures are necessary.The Project <br /> Manager and/or Site Safety Officer will review daily reports provided by the field team member. The field <br /> team member(s)will report daily project activities to the Project Manager and Site Safety Officer. The Site <br /> Safety Officer or Alternate Site Safety Officer will be immediately available(either onsite or at the project <br /> command post)whenever personnel are in the Exclusion Zone of any work area. <br /> The Site Safety Officer is responsible for ensuring that the provisions of this health and safety plan are <br /> adequate and properly implemented in the field. The Site Safety Officer has the authority to correct any <br /> health and safety problems encountered during this project. <br /> 1.2.5 SALEM Field Team Members <br /> The responsibilities of the SALEM field team member with respect to safety are to: <br /> • Become thoroughly familiar with this safety plan; <br /> • Actively participate in this project health and safety plan; <br /> • Follow safety standards and safe work practices set out by SALEM, the client, and regulatory <br /> agencies; <br /> • Refuse to perform work when unsafe conditions exist; <br /> • Report potential hazards to the Project Manager; <br /> • Immediately report all potential hazards, accidents, incidents, injuries, and illnesses to the Project <br /> Manager and Corporate Health and Safety Manager; <br /> • Use personal protective equipment when needed; <br /> • Check equipment and tools,including personal protective and safety equipment,for hazards before <br /> use; and <br /> • Know the location,type, and operation of all site and facility emergency equipment. <br /> SALEM <br /> engineering group. inc. <br />
