Box 806, Sacramento, CA 95812-0806 <br /> CORRECTIVE ACTION: Locate a copy of the destination facility signed manifests and send a copy to the San <br /> Joaquin County Environmental Health Department(EHD). If the destination facility signed manifests cannot be <br /> found then prepare an exception report for each missing manifest as required by Title 22 California Code of <br /> Regulations section 66262.42. The exception report shall include a legible copy of the manifest for which the <br /> generator does not have confirmation of delivery and a cover letter signed by the generator or the generator's <br /> authorized representative explaining the efforts taken to locate the hazardous waste and the results of those efforts. <br /> DTSC address is DTSC Report Repository, Generator Information Services Section, PO Box 806, Sacramento, CA <br /> 95812-0806.
