APPENDIX C <br /> Diamond Pet Food Processors of Ripon,LLC June 30,2011 <br /> Report of Waste Discharge—Groundwater Sampling Plan Page 4 of 7 <br /> To comply with packaging regulations and to take practical measures to prevent damaging samples, <br /> the sampling personnel will follow packaging and shipping instructions supplied by the certified <br /> testing laboratory. <br /> The chain of custody form for the samples in the container will be placed inside the cooler. On <br /> receipt, the laboratory will note on the chain of custody form the temperature of the samples and any <br /> evidence of damage or tampering. <br /> SAMPLING DOCUMENTATION <br /> It is essential that all sampling activities be documented on a FSDS. Entries should be made in <br /> indelible ink and changed by crossing out the entry with a single line and initialing it. The data <br /> recorded on a FSDS depends on the sampling activity being performed and will include some or all <br /> of the following: <br /> • Facility name; <br /> • Date; <br /> • Sampler's name and signature; <br /> • Sample type; <br /> • Sample identification; <br /> • Weather(ambient temperature, wind condition, etc.); <br /> • Field instrument calibration; <br /> • Field measurements (water level, pH, conductivity,temperature, and turbidity; <br /> • Field calculations (such as well purge volumes); <br /> • Number, volume, and type of sample containers; <br /> • Condition of the sample location(monitor well, riser pipe, etc.); and <br /> • Any unusual conditions that may affect samples or any deviations from the normal sampling <br /> protocol. <br /> To help maintain the integrity of the samples, strict chain-of-custody procedures will be <br /> implemented. These procedures will help ensure that tampering of samples does not occur. The <br /> chain-of-custody form is a manifest of the sample containers and describes the analyses to be <br /> performed on the samples in each container. All sample containers will be clearly and <br /> unambiguously labeled and chain of custody forms will be completed before the end of each <br /> sampling day. A written record of the sample bottle possession and any transfer of the samples must <br /> be maintained and documented on the chain of custody record. The chain-of-custody forms and <br /> relevant FSDSs will be sent with the samples to the laboratory.
